New Business 3. 25-109 Consideration of Fiscal Year 2025-2026 Health and Human Services funding recommendations to be presented to City Council at the April 14, 2025, Work Session. Commissioner Moeller moved,seconded by Vice Chair Danford that funding for Health and Human Services for Fiscal Year 2025-2026 be as follows: Apache Junction Community Development Corporation(AJ CDC) in the amount of$7,000.00 for overall operating expenses. Begin Again Homes in the amount of$2,000.00 for emergency housing for Apache Junction residents. Boys and Girls Club of the Valley in the amount of$9,000.00 for the mentorship program (6,000.00) and the hardship to individuals program($3,000.00). Over-Flo Missions in the amount of$2,000.00 for emergency food boxes or hot meals for Apache Junction residents. Superstition Community Food Bank in the amount of$70,000.00 for the purchase of emergency food($50,000.00)and to expand the Student Backpack Program to 2 new schools($20,000). Yes: 7- Commissioner Brennan, Commissioner Moeller, Commissioner Danford, Commissioner Smithson, Commissioner Wood, Commissioner Voisine and Commissioner Mykland No: 0 Chairperson Brennan asked City Attorney Joel Stern to clarify the funding process per the City Code requirements in an email prior to this meeting. He summarized the response for the Commission members and for the record. The City Code requires non-profit organizations to show the use of funding to benefit residents rather than non city residents. The exception to this rule is the Food Bank, as they provide statistical reporting in a less specific manner so that privacy rights are not compromised. Commission members stated their individual recommendations and reasoning behind them. They debated on the amounts for each non-profit and landed on 3 options for the final funding recommendation. Representatives from the Food Bank, Begin Again Homes, Over-Flo Missions, and the Boys and Girls Club each spoke with the Commission and answered additional questions. The Commission agreed that option 3 was the best choice for Fiscal Year 2025-2026 Funding. G. Information and Reports H. Staff Liaison's Report There was no Staff Liaison report. The following provides instructions and eligibility information regarding the request for non-profit funding. The commission was formed under city code Article § 2-11. This program is made possible under city code Article § 2-14 Request for Financial Assistance. The city code is available at https://www.apachelunctionaz.gov/ancode. Per city code, the request for financial assistance must be submitted to the city clerk's office during the month of January to be considered for the following fiscal year. All funding is subject to the execution of an agreement with the city, the approval of a budget allocating funds to the program, and the city council's approval. Eligible applicants must: 1. Be a federal recognized 501 c 3 2. Serve Apache Junction Proper Residents; and 3. Submit a completed application by the published due date Eligible Activities: Programs and services affording health, public health, and human services care to all people, especially, to individuals of underprivileged conditions. Populations Served: Elderly, handicapped, developmentally disabled, and other special populations. Priority Projects: The Commission reserves the right to identify priority projects for funding. City of Apache Junction,Arizona Pagel Printed on 112212026 §2-11-5 DUTIES. The Health and Human Services Commission is hereby charged with the following duties: (A) Conduct public hearings to determine the needs of city residents with regard to public health provision of human services and care of the elderly, and disabled; (B) Enhance the development of the public health through public forums, seminars and work with other agencies; (C) Review requests submitted by health and human service providers (non-profit agencies)for city funding and to make recommendations to the Council regarding that funding; (D) Assist in the development of health care facilities; (E) Make recommendations to the Council regarding elderly health care and emergency transportation programs. (Prior Code, §2-11-5) (Am. Ord. 1519, passed 12-7-2021) 0§ 2-14-1 REQUESTS FOR NON-PROFIT FUNDING. (A) Required documentation. Any non-profit agency submitting a request for an allocation or grant of city funds shall submit to the city specific documentation including,but not limited to, the following: 1. A copy of the agency's most recent audit report; 2. a certified copy of the agency's most recent financial statement; 3. detailed expenditure statements in order to provide a detailed accounting of all funds previously received from the city; 4. complete information on the source and amount of funding received from all other sources such as non-governmental agencies, 5. membership fees and dues, and private contributions; 6. client service information as it applies to residents of the city; 7. proof of non-profit status as determined by the Internal Revenue Service; 8. proof of corporate status to include copies of by-laws and articles of incorporation; 9. the source and amount of funding received from other governmental agencies; 10. the names and addresses of current board members; 11. and any other documentation as may be deemed necessary by the city. (B) Reporting requirements. Any non-profit agency receiving an allocation or grant of city funds shall be required to submit to the city, on a quarterly basis, a detailed accounting of the expenditure of city funds for the previous quarter, a written report outlining the agency's performance and accomplishments within the scope of work outlined in their contractual agreement with the city, and any other documentation as may be deemed necessary by the city in order to determine the agency's compliance with the provisions of the contract. (C) Contract required;procedure for distribution of funds. (1) All city funds allocated or granted to any non-profit agency shall be by means of a written contract based upon services to be provided to or work to be performed on behalf of the city and its residents in compliance with the provisions of Arizona Revised Statutes regarding the use of public funds. (2) All city funds allocated or granted under the provisions of this section shall be released in equal quarterly installments or quarterly payments based upon a schedule of anticipated expenses which has been approved by the Mayor and Council. No subsequent quarterly allocation or grant shall be released until such time that the receiving agency has provided all of the required documentation for the previous quarter and has provided satisfactory evidence of compliance with the scope of work stipulated in their contract with the city. (D) Submittal of requests for funding. All funding requests submitted by non-profit agencies for the allocation or grant of city funds shall be submitted to the City Clerk's office during the month of January of each year. Requests so submitted shall not be subject to city funding unless approved by the Council and only following adoption of a final budget for the subsequent fiscal year. (Ord. 659,passed 11-3-1998) 0§ 2-14-2 REQUESTS FOR FINANCIAL ASSISTANCE. All requests for financial assistance shall comply with the following stipulations and conditions: (A) All requests shall be filed by or on behalf of a valid, non-profit organization as qualified by the Internal Revenue Service and as registered with the Arizona Secretary of State, Arizona Corporation Commission or other appropriate state office. The registration shall be current and documentation of such status and registration shall be provided at the time of the request. This article shall not apply to governmental or quasi-governmental jurisdictions; (B) All requests shall be submitted to the City Clerk during the month of January of each year in order to be eligible for consideration in conjunction with the subsequent fiscal year budget; (C) All requests received in accordance with division(B) above shall be referred to the appropriate city board or commission for purposes of review and recommendation to the Council; and (D) In those instances where a request is due to catastrophic circumstances or when the public health, safety and welfare is at risk, the requirements of this article may be waived by action of the Council. It is preferable, however, even in such instances, for the request to receive a review and recommendation from the appropriate city board or commission. BGCAZ's Power Hour homework assistance program has led to improved academic performance,with 95%of participating members reporting better grades and increased understanding of their schoolwork. High school graduation rates among members have risen,with 90%of seniors graduating on time, surpassing the state average. Over 70%of these graduates pursue post-secondary education. In Quarter 3 at the Superstition Mountain Branch,43 members attended Power Hour, and this program was provided on 36 days during the quarter at the Club. • Healthy Lifestyle choices Through BGCAZ's Healthy Lifestyles Priority Outcome,members engage in daily physical activity, make healthy nutrition choices,employ good resistance and decision-making skills,and refrain from participating in risky behaviors. In Quarter 3 at the Superstition Mountain Branch, 429(duplicated) members participated in Healthy Lifestyles programs! Healthy lifestyles programming was provided 53 days throughout Quarter 3.We are excited to continue this programming at this Branch. • Leadership and Character building through Torch&Keystone Club BGCAZ's priority outcome, Good Character&Leadership,is predominantly emphasized through our flagship programs Torch Club(for 6th-8th graders)and Keystone Club(for 9th-12th graders). The Torch Club at the Superstition Mountain Branch is chartered leadership and service clubs for middle school youth,through which Club staff can help meet the special character development needs of younger adolescents at a critical stage in their development.Torch Club members learn to elect officers and work together to implement activities in four areas: service to Club and community,education, health and fitness,and social recreation.The program helps them develop leadership skills, practice teamwork, and foster a sense of responsibility while they learn to make good decisions, set goals,and develop beneficial life skills. Keystone Club is BGCAZ's most dynamic teen program. It affords teens the opportunity to gain valuable leadership and service experience, explore career paths,get involved in their Club and community, and connect with other teens.The program is designed to help high school aged youth grow as community members and increase their confidence as these teens conduct activities in four areas: academic success, career exploration, community service, and teen outreach. In Quarter 3, 10 members attended the Keystone program, and 10 members attended the Torch Club Program. Keystone was provided for 5 days while Torch Club was provided for 7 days. • Career Pathways and workforce readiness BGCAZ's Career Pathways and Workforce Readiness allow Club members to engage in programs where teens can explore career and internship opportunities in a wide variety to develop their own personal plan for success. BGCAZ wants to give all youth and teens the skills they need to be impactful leaders of their community. In Quarter 3, 12 members workforce readiness programs, and these programs were offered for a total of 12 days.
