A RESOLUTION PROVIDING FOR THE EXECUTION OF
APPLICATION AND AGREEMENT BETWEEN THE CITY OF APACHE
JUNCTION AND THE ARIZONA STATE RETIREMENT SYSTEM BOARD
FOR INCLUSION OF ELIGIBLE OFFICERS AND EMPLOYEES OF THE
CITY IN THE SUPPLEMENTAL RETIREMENT PLAN.
WHEREAS the City of Apache Junction,Arizona ( "City")
through its City Council desires to make formal application to the Arizona
State Retirement System Board ("Board") to include all eligible officers
and employees of the City in the Supplemental Retirement Plan provided
by Article 2.1,Chapter 5 of Title 38,Arizona Revised Statutes;and
WHEREAS the City desires to file such an application with
the Board and agrees to b e bound by the terms and conditions set forth
in the application and agreement;and
WHEREAS a form of application and agreement between the Board
and the City containing the terms and conditions for inclusion by the Board
of the said eligible employees for whom coverage is requested has been
submitted to this body for consideration and action;
NOW,THEREFORE,BE IT RESOLVED that the City execute the
aforesaid application and that the Board be,and it is hereby requested
to include all such eligible employees of the City for whom coverage is
requested in the said Supplement Retirement Plan.

NOW, THEREFORE, BE IT RESOLVED that the City
