A RESOLUTION OF THE MAYOR AND CITY COUNCIL OF THE CITY OF APACHE
JUNCTION,ARIZONA,CONCERNING THE ARIZONA MUNICIPAL WORKERS'
COMPENSATION POOL ("POOL").
BE IT RESOLVED BY THE MAYOR AND CITY COUNCIL OF THE CITY OF APACHE JUNCTION,
ARIZONA,AS FOLLOWS:
WHEREAS,the City Council of the City of Apache Junction has decided to join the
Arizona Municipal Workers'Compensation Pool ("Pool").
THEREFORE,BE IT RESOLVED that the City of Apache Junction through adoption of
this resolution by its City Council hereby authorizes Southwest Risk Services,
Inc.,the ("Pool")administrator,to sign the Industrial Commission of Arizona
(ICA)application required to notify the ICA of ("Pool") membership.

JUNCTION,ARIZONA, THIS 17TH DAY OF JUNE ,1997.
